The Care OperationsCoordinator reports directly to the Executive Director, ClinicalResources.This position supports everyservice line of the hospital in conjunction with the Utilization Reviewdepartment to support the efficiency of the use of health care services.In addition, this position also directlysupports the Social Work Services department and the Palliative Careprogram.The incumbent of this positionwill be greatly responsible for the administrative aspects of each department'soperations.Working directly with dischargeplanners, nursing personnel, licensed clinical social workers, and nursepractitioners to assist in organizational operations to support compliance withregulatory requirements.Finally, thisposition is also focused on improving the quality of care we provide throughaudits, analysis, and continuous improvement.
Essential Functions and Responsibilities of the Job
Provides overall administrative support forthe assigned services areas including Utilization Review, Social Work Services,and Palliative Care to ensure all patient care needs are met.
Conducts departmental auditing, accountspayable, daily / monthly schedules, and coordination of meeting and officeactivities for three cost centers / departments, to meet the needs of thehospital and staffing requirements.
Generates daily patient assignments forUtilization Review department, assigning RN Care Managerspatient caseloads, to ensure the needs forpatient care are addressed in a timely manner, Executive Director and / or Leadprovides input in finalizing the patient assignment.
Develops and maintains systems anddatabases to support department needs, such as: National Palliative CareRegistry, Charity Contributions, Intranet Departmental Website, ComplianceAudits
Develops and maintains systems and databasesto support required department analytics.
Assists with project plans under leadershipguidance, to identify key issues, address gaps in process, and improve onperformance metrics.
Effectively communicates with all levels ofhospital staff in all service areas to assist in facilitating positive andefficient patient outcomes.
Demonstrates the ability to activelyparticipate in departmental activities, attend relevant meetings, and functionas a key contact for Utilization Review, Social Services, and PalliativeCare.
Understands the importance of HIPAA Privacyand Security Regulations associated with electronic protected healthinformation (ePHI) and remains in compliance during daily operationalactivities.
Assists in assessing service area challengesand daily issues to identify root causes, gather relevant information,collaborate on possible solutions, and make recommendations in an effort tosolve problems and meet goals.
Actively assists with maintainingregulatory accreditation and administrative operation documentation to fulfillany request and or compliance standard.
Triages phone calls / messages for RN, CaseManagers for clinical reviews to the appropriate staff to ensure timely andaccurate information is received.
