The Health Information Management (HIM) Associate II is responsible and accountable for a wide variety of functions within the HIM Department. These functions can include the following: assembling and assigning chart deficiencies; gather and submitting birth certificate information; providing follow-up activities on the behalf of the Cancer Registry; receiving and researching issues with transcription; filming or scanning images for record retention; and researching and locating medical records for internal needs of the HIM Department. The specific functions assigned depend on complexity of the facility and its staffing needs.
ESSENTIAL FUNCTIONS OF THE ROLE Answers and directs phone calls concerning transcription services.
Routes and assigns transcription jobs based on priority to the Transcriptionists.
Assigns medical records to appropriate locations and locates medical records using the automated chart tracking system.
Assembles and assigns deficiencies using the chart management system to track and monitor physician?s incompletion activities.
Provides coverage in the Physician?s incomplete area and answers Physician questions, as necessary.
Microfilms or scans documentation for the purposes of record retention.
Interacts with new parents to ensure birth certificate information is collected and submitted to the Texas Department of Health in a timely manner.
Uses a variety of resources to provide Cancer Registry follow-up functions and maintain the follow-up percentage as required by American College of Surgeons standards.
Researches and prioritizes accounts for coding. These accounts will be located and routed to the HIM Coders. Charts that have been coded will be routed to the next appropriate location within the HIM department.
Researches and files appropriate documentation on medical records as required by position.
Works closely with direct Manager to ensure operational needs of facility are met.
Utilizes the chart management system as required by task and responsibility.

- EDUCATION - H.S. Diploma/GED Equivalent
- EXPERIENCE - 1-3 months on-the-job training required
