Providence Medical Group (PMG), in partnership with Providence Senior Health, is seeking a full-time Geriatrician to develop and lead an outpatient geriatrics consultation program in Portland, Oregon. You will be joining a patient-centered medical home team of physicians, advanced practice clinicians, a behaviorist, clinical pharmacist, case manager, registered nurse, and certified medical assistants to provide exceptional coordinated care to older adults and their families. The vision for this position is to provide more clinical expertise to our older adult patients seen by our primary care providers, whether it is in person, virtually or in an e-consult mode. Your clinical expertise will especially guide outpatient consultations on cognitive concerns as part of an integrative primary care model referred by our PMG primary care providers. You will also become part of a geriatric clinical community that has been created through the development of a Geriatric Mini-Fellowship, an educational partnership between Senior Health and PMG. This program supports Primary Care providers in becoming Age-Friendly champions within their clinics. In this role, you will have clinical time as well as time dedicated to program and care pathway development with the Senior Health team. Additionally, to continue to progress Age-Friendly provider training, you will have time established to partner with a dedicated Geriatric Educator in support of Providence Oregon's three residency programs (IM and FP) in the Portland metro area. Senior Health has implemented multiple services to support PMG in establishing Age Friendly practices and programs and has been recognized by the Institute for Healthcare Improvement (IHI) as a Pioneer Age-Friendly Healthcare System. Senior Health supported programs include a robust Falls Risk Management Program, a Dementia Care Ecosystem in collaboration with UCSF, and a Geriatric Mini-Fellowship to upskill primary care providers across Providence. • Board-certified or Board-eligible in geriatric medicine
• Full-time (0.8 FTE - 1.0 FTE)

• Development of a Memory Concern Clinic and care pathways in partnership with aligned clinical services

• Ideal candidates will have an interest in teaching and innovative program development
• Two years of recent geriatric clinical experience advised
