SummaryThis position is located in the Primary Care Service - at the Eastern Colorado Health Care System and provides support to the primary care of Veterans and their significant others within the ECHCS catchment area. Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model).
